By signing your child up you for Junior All Stars you agree to the following conditions. I acknowledge and accept that my child will remain under the care, direction, and supervision of the school while receiving instruction from Junior All Stars LLC. I understand that participating in sports, both indoors and outdoors, carries inherent risks of serious physical injury, exposure to disease or illness, and even death.
On behalf of myself and my child, I assume full responsibility for all risks associated with my child’s participation in any classes, programs, and activities offered by or on behalf of Junior All Stars LLC. By signing this waiver, I forever release, discharge, and hold harmless Junior All Stars LLC, the Childcare Facility, and all related parties, including owners, employees, affiliates parents, subsidiaries, affiliated companies, successors, and sponsors. This release covers any claims, actions, damages, losses, liabilities, costs, and expenses arising from my child’s participation in any program, whether due to negligence or willful misconduct.
I also agree not to initiate any legal action or claim against the Released Parties based on my child’s participation in any program, regardless of the legal theory (including personal injury, negligence, rights of privacy, and defamation). Furthermore, I will indemnify, defend, and hold harmless the Released Parties from any loss, liability, cost, claim, or damages, including reasonable attorney’s fees, related to my child’s participation in or association with the programs.
Please note you can only sign your child up for Junior All Stars if they are enrolled in any of the schools listed below. If you sign your child up and we do not work with any of the schools listed below your payment will not be refunded.
By signing your child up, you agree to the following policy regarding tuition payments. Tuition payments are due monthly. Late or missed payments received could prevent participation in classes. You will be emailed a pay link to renew tuition to the email address we have on file 14 days before tuition is due. Please contact us if you wish to make multiple payments for multiple months upfront. If a payment is missed there will be a one class grace period and a notice to pay will be sent home directly to the email provided to Junior All Stars LLC. Please resolve all notices immediately to ensure smooth enrollment and avoid any penalties. If you have a change in email address or contact information, please contact us immediately to ensure proper notices are sent to your child's guardian.
If the payment has not been received after the grace period and before the next scheduled class period, your child will be removed from Junior All Stars, and you will be billed for the whole month. If you wish to re-enroll your child in Junior All Stars all outstanding payments will need to be paid in full before re enrollment. Please contact us immediately if you have an outstanding balance with Junior All Stars before re enrollment. If you fail to do so we will apply any payment towards outstanding balances and your child will remain unenrolled in Junior All Stars.
By signing your child up for Junior All Stars, you agree to the following policies regarding refunds and cancellations. There are no refunds for absences or for classes that fall on holidays or planned school closures.
In order to unenroll your child from Junior All Stars please contact us directly using the contact us tab on our website. A 10 Calander days' written notice before the next billing period is required to withdraw from the program. Please note the 10 Calander days does include any weekends or holidays. There are no refunds for classes missed due to improper notice given or failure to give notice. A notice given to your child's coach or child's school will not be sufficient.
In the event that a scheduled class is cancelled for non-holiday or school closure circumstances, every effort will be made to makeup that class as soon as possible. If a makeup class cannot be held the same week, future classes may be extended to makeup the time. If extending class durations are not an option, we may try and perform multiple classes in one week however no more than two classes will be performed in a one-week period. Refunds will be made on a case-by-case basis.
By signing my child up for Junior All Stars, I acknowledge that Junior All Stars LLC and its representatives may take photographs or images of my child during scheduled class time. I acknowledge photos will be used for website and social media purposes. I understand that the photographs or images may be used in various media formats, including but not limited to, digital, and online platforms. I acknowledge that all photos of my child that Junior All Stars LLC wishes to use for the purposes listed above must include a proof and explanation for use as well as my approval in writing before use.
