Photo Release

By signing my child up for Junior All Stars, I acknowledge that Junior All Stars LLC and its representatives may take photographs or images of my child during scheduled class time. I acknowledge photos will be used for website and social media purposes. I understand that the photographs or images may be used in various media formats, including but not limited to, digital, and online platforms. I acknowledge that all photos of my child that Junior All Stars LLC wishes to use for the purposes listed above must include a proof and explanation for use as well as my approval in writing before use. By signing up I agree to the follow conditions listed below

• I am voluntarily granting permission for the following information stated above.
• I waive any right to compensation or royalties for the use of these photographs or images.
• I release Junior All Stats LLC from any liability related to the use of these photographs or images.
